Job Summary

Under the general supervision of the Rehab Manager, provides direct speech therapy services to patients in their homes and in other community settings.  S/he consults with VNA staff and community agency personnel to provide instruction to patients, families, staff and other appropriate persons.

Job Responsibilities and Functions

The function of the Speech & Language Pathologist is to provide direct speech therapy services in accordance with a plan of treatment which the speech therapist helps to develop;  revise care plan as necessary in collaboration with the patient/family, the NVNA health care team which is approved by individual patients’ physicians.

Operational and Care Coordination Functions

  • Respect the rights of the patient to informed decisions regarding care when differences exist between the patient needs, applicable laws and regulations and/or the agency’s capability or mission.
  • Consider social and environmental factors affecting the patient’s condition and incorporates these factors into the care plan. Insure social, cultural, religious and ethical issues/conflicts are resolved involving the patient, family, physician and appropriate NVNA staff.
  • Establish realistic measurable goals when implementing care plans involving the patient/family in all aspects of care.
  • Conduct assessments, develop care plans, determine goals and interventions specific to the age and developmental level of the patient.

  • Regularly evaluate patient care plans and goals and adjust plans in collaboration with the patient according to patient progress.
  • Facilitates accurate, timely and efficient documentation of reimbursed services

Qualifications

  • Graduation from an accredited speech therapy program
  • Licensure to practice as a registered speech therapist in Massachusetts
  • Accreditation for CCC in addition to current licensure
  • Minimum of one year speech therapist experience as a clinician
  • Homecare experience preferred
  • Current CPR certification
  • Current valid driver’s license, reliable transportation and current auto insurance

  • Regularly evaluate patient care plans and goals and adjust plans in collaboration with the patient according to patient progress.

Performance Improvement Responsibilities and Functions

  • Promotes customer service by responding to and resolving difficult and sensitive patient and staff inquiries and complaints

Personal and Professional Development

  • Provide in-service education for agency staff and other community agency personnel.
  • Offer community programs in area of expertise as requested and as time permits.
  • Maintain current knowledge of speech therapy practices and acquires new skills as needed by attendance at workshops, courses, in-service education sessions, etc.
  • Assist with orientation and training of newly-hired speech therapists.
  • Participate in the agency Quality Assurance/Improvement activities.

Working Conditions

  • Potential exposure to a variety of diseases which may be communicable in nature
  • Latex exposure can be expected as some environments are not latex free
  • May travel from office to meetings, patient homes and other facilities for extended periods of time and in a variety of weather conditions
  • Some non-standard work hours and all-call are required
  • Additional certifications may be required, including basic life support depending on work area

Minimum Physical Requirements

  • Standing, sitting walking, bending, squatting, repetitive movements, driving
  • Weight lifting up to 50 pounds and weight transferring up to 150 pounds
